Has your doctor, spouse, or children asked if you have an Advance Care Directive? This essential document ensures your medical wishes are known if you can’t speak for yourself.
Join attorney Blaire McClanahan for a free session to learn what an Advance Care Directive is, what it does (and doesn’t) do, and why it’s important. She’ll guide you through completing your own form, which will be notarized on site.
What you need to do:
- Register (no walk ins, space is limited)
- Bring a current ID
- Have the correct spelled name of the person you would like to make medical decisions on your behalf. You will also need their address, cell phone and home phone if applicable.
